How long does retatrutide take to work? In trials, changes in blood sugar and weight showed up within the first 12 weeks, but the full effect took much longer. In the phase 2 obesity trial, weight was down 17.5% at 24 weeks on 12 mg and 24.2% at 48 weeks, and it was still falling.[1] Phase 3 trials ran for 80 weeks, and some people kept losing weight to 104 weeks.
Retatrutide is an investigational Eli Lilly drug and is not FDA-approved. The timelines below come from controlled trials. For the bigger picture, see the retatrutide profile and retatrutide weight loss results.
The short version: a timeline from the trials
| Time on treatment | What the trials showed |
|---|---|
| Days to weeks | After a single injection, weight reduction lasted up to day 43 |
| Weeks 0 to 16 | Dose stepped up every 4 weeks; 12 mg reached at week 16 |
| Week 12 | Lower blood sugar and weight on the higher doses (phase 1b) |
| Week 24 | Weight down 7.2% to 17.5%; liver fat down by up to 82.4% |
| Week 40 | HbA1c down 1.69 to 1.94 points (TRANSCEND-T2D-1) |
| Week 48 | Weight down 8.7% to 24.2% (phase 2) |
| Week 80 | Weight down 17.6% to 25.0% (TRIUMPH-1) |
| Week 104 | Up to 30.3% in a selected extension group |

Why the first months are a ramp-up
Retatrutide is not started at the full dose. In the phase 3 TRIUMPH trials, people began at 2 mg once weekly and moved up every four weeks until they reached their assigned dose:[4]
| Target dose | Steps | Week target dose begins |
|---|---|---|
| 4 mg | 2 mg, then 4 mg | Week 4 |
| 9 mg | 2, 4, 6, then 9 mg | Week 12 |
| 12 mg | 2, 4, 6, 9, then 12 mg | Week 16 |
The trial design paper describes this as 16 weeks of dose escalation followed by 64 weeks of maintenance in the 80-week trials.[5] The slow climb is there to limit stomach side effects: in phase 2, starting at 2 mg rather than 4 mg partly reduced them.[1]
So much of the weight loss in the trials happened after people had reached their full dose. Early weeks on a low dose are not a fair test of what the drug can do. The retatrutide dosage guide covers each trial’s doses.
The first 12 weeks
The earliest human data are small but show the drug acting quickly:
- One dose. In the first study in healthy volunteers, a single injection reduced weight, and the reduction persisted up to day 43 afterward.[3]
- Twelve weeks. In the phase 1b study of 72 people with type 2 diabetes, the three highest dose groups had lower average daily blood sugar and lower HbA1c than placebo at week 12. Weight loss appeared dose-dependent, up to about 9 kg more than placebo in the group that stepped up to 12 mg.[2]
The published trial summaries do not give a week-4 or month-1 weight figure, so we cannot say how many pounds people typically lost in the first month.
Weight loss at 24 and 48 weeks
The 48-week phase 2 obesity trial in 338 adults gives the clearest picture of the time course:[1]
| Dose | Week 24 | Week 48 |
|---|---|---|
| 1 mg | −7.2% | −8.7% |
| 4 mg | −12.9% | −17.1% |
| 8 mg | −17.3% | −22.8% |
| 12 mg | −17.5% | −24.2% |
| Placebo | −1.6% | −2.1% |
Two things stand out. Most of the loss on 1 mg had happened by week 24. On 4 mg and above, people lost another 4 to 7 percentage points between week 24 and week 48. By week 48, the share of people who had lost at least 15% of their body weight was 60% on 4 mg, 75% on 8 mg and 83% on 12 mg, compared with 2% on placebo.[1]
Results at 80 and 104 weeks
The phase 3 trials measured their main result at 80 weeks. In TRIUMPH-1, average weight change was −17.6% on 4 mg, −23.7% on 9 mg and −25.0% on 12 mg, compared with −3.9% on placebo, counting everyone randomized.[6]
A pre-specified extension followed 532 people who started with a BMI of 35 or more, finished 80 weeks and tolerated their dose. Those originally on 12 mg reached an average loss of 30.3% at 104 weeks.[7] Because this group was selected for staying on treatment, it does not represent everyone who starts the drug.
People with type 2 diabetes lost less over the same 80 weeks: −11.9% to −18.8% in TRIUMPH-2, versus −5.1% on placebo.[11]
Blood sugar and liver fat
Not every benefit takes 80 weeks:
- Blood sugar. In the phase 2 diabetes trial, HbA1c fell by up to 2.02 percentage points at 24 weeks on 12 mg, and results held at 36 weeks.[8] In the phase 3 TRANSCEND-T2D-1 trial, HbA1c fell by 1.69 to 1.94 points by week 40, versus 0.81 on placebo.[9]
- Liver fat. In a substudy of people with fatty liver disease, liver fat fell by 81.4% on 8 mg and 82.4% on 12 mg at 24 weeks, and 86% of people on 12 mg reached normal liver fat. The drop tracked with weight and belly-fat loss.[10]
- Knee pain and sleep apnea. TRIUMPH-1 measured these at 80 weeks, when both had improved more than on placebo.[6]
Side effects follow their own timeline
- Stomach side effects were most common while doses were rising. In TRANSCEND-T2D-1 they were mostly mild to moderate and subsided over time.[9]
- Heart rate rose with dose in phase 2, peaked at 24 weeks and then declined.[1]
See retatrutide side effects for rates by dose.
Why results vary
- Dose. Higher doses produced more weight loss, though the gain from 9 mg to 12 mg was small in TRIUMPH-1.[6]
- Diabetes. People with type 2 diabetes lost less weight.[11]
- Staying on treatment. Published results count everyone randomized, including people who stopped. Lilly’s estimate for people who stayed on 12 mg in TRIUMPH-1 was 28.3% at 80 weeks, versus 25.0% counting everyone.[6][7] The gap reflects people who stopped early.
- Lifestyle support. Everyone in the TRIUMPH trials, including the placebo groups, received counselling on healthy eating and physical activity.[5]
- Product. Trial timelines apply only to Lilly’s drug. FDA says products sold as “research” retatrutide are of unknown quality and that retatrutide cannot be used in compounding.[12] With an unverified product, no trial timeline applies.
For how these timelines compare with approved drugs, see how long tirzepatide takes to work and how long semaglutide takes to work.
Questions to ask a clinician
- How long should I give an approved medicine before we judge whether it is working?
- What should I expect while the dose is being increased?
- Which measures besides weight, such as HbA1c or liver tests, should we track?
- Would I qualify for a retatrutide clinical trial?
