Quick answer: how long does semaglutide take to work? Each injection reaches peak blood levels within 1 to 3 days, and levels hold steady after 4 to 5 weeks on the same dose.[1] Weight loss builds over months: about 10% of body weight by week 20 and about 15% by week 68 in the STEP trials of Wegovy 2.4 mg.[3][6]
The slow start is mostly on purpose. Every semaglutide product begins at a low dose and steps up over 16 weeks or more to limit nausea. For the full schedules, see our semaglutide dosage chart. For the drug itself, see the semaglutide profile.
How quickly semaglutide gets into your blood
After an under-the-skin injection, semaglutide reaches its highest level 1 to 3 days later, and about 89% of the dose is absorbed.[1] With a weekly drug, levels build up over several doses. The Ozempic label says steady state, the point where each week’s dose balances what your body clears, is reached after 4 to 5 weeks of weekly dosing.[1]
That is why each dose step lasts at least 4 weeks: your body needs about that long to settle at each level. The Wegovy pill works differently. It peaks about 1 hour after you take it, but because you take it every day, it also builds up over weeks.[2]
How long semaglutide takes to suppress appetite
Semaglutide reduces appetite by acting on GLP-1 receptors that affect hunger and fullness. The Wegovy label says it lowers calorie intake, probably by changing appetite.[2]
The clearest data comes from a 20-week study of 72 adults with obesity:[4]
- At a test lunch where people could eat as much as they wanted, those on semaglutide 2.4 mg ate 35% fewer calories than those on placebo.
- They reported less hunger, more fullness and fewer or weaker food cravings.
- They lost 9.9% of their body weight, compared with 0.4% on placebo.
That study measured appetite at week 20, not in the first days. Trials have not measured appetite well in the first few weeks, when people are still on the low starting doses. So there is no reliable figure for how soon hunger drops, and it is reasonable to expect a bigger effect once you reach higher doses.
How long semaglutide takes to lower blood sugar
For type 2 diabetes, semaglutide helps the pancreas release insulin when blood sugar is high, and lowers glucagon, a hormone that raises blood sugar.[1]
- 12 weeks: in the label’s mechanism studies, Ozempic 1 mg lowered fasting glucose by 29 mg/dL and the 2-hour after-meal glucose by 74 mg/dL compared with placebo.[1]
- 30 weeks: in SUSTAIN 1, HbA1c (a 3-month average of blood sugar) fell by 1.45 percentage points on 0.5 mg and 1.55 points on 1 mg, compared with almost no change on placebo. Body weight fell by 3.7 to 4.5 kg.[5]
Your clinician usually checks HbA1c about 3 months after a change, because that is how long the test takes to reflect it.
Semaglutide weight loss timeline by week and month
These results come from different trials, doses and groups of people, so treat them as a rough timeline, not a forecast for any one person.
| Time on treatment | What happened | Source |
|---|---|---|
| Weeks 1-16 | Dose steps up from 0.25 mg to 1.7 mg; 2.4 mg starts at week 17 | Wegovy label[2] |
| Week 20 | About 10.6% average weight loss (STEP 4 run-in) | STEP 4[3] |
| Week 20 | 9.9% weight loss and 35% lower intake at a test meal | Appetite study[4] |
| Week 64 | 13.6% weight loss on the 25 mg Wegovy pill vs 2.2% on placebo | OASIS 4[7] |
| Week 68 | 14.9% weight loss on 2.4 mg vs 2.4% on placebo | STEP 1[6] |
| Week 72 | 18.7% weight loss on 7.2 mg vs 15.6% on 2.4 mg | STEP UP[8] |
In STEP 1, 86% of people on semaglutide lost at least 5% of their weight by week 68, 69% lost at least 10%, and about half lost at least 15%.[6] That leaves a meaningful share who lost less. Our guide to not losing weight on a GLP-1 covers common reasons.
Ozempic timeline
Ozempic is approved for type 2 diabetes, not weight loss, but it contains the same drug as Wegovy at lower maximum doses.[1] A typical Ozempic timeline looks like this:
- Weeks 1-4: 0.25 mg, mainly to let your body adjust.
- Weeks 5-8: 0.5 mg, the first maintenance dose.
- Week 9 or later: 1 mg if more blood sugar control is needed, then 2 mg after at least 4 more weeks if needed.[1]
Blood sugar results are usually judged at about 3 months. Weight loss on Ozempic doses tends to be smaller than on Wegovy 2.4 mg; in SUSTAIN 1, people lost about 4 to 5 kg over 30 weeks.[5]
Wegovy pill timeline
The Wegovy pill follows a slower, 90-day ramp: 1.5 mg daily for 30 days, then 4 mg, then 9 mg, reaching the 25 mg maintenance dose on day 91.[2] Each tablet peaks in the blood about an hour after you take it, but daily dosing builds up gradually, and at 25 mg the average steady-state level is similar to the 2.4 mg weekly injection.[2] In OASIS 4, the 25 mg pill led to 13.6% average weight loss at 64 weeks.[7] Taking it correctly matters: on an empty stomach, with up to 4 ounces of water, and nothing else for 30 minutes.[2]
Why it takes months, not days
- Titration: the low starting doses cut side effects but also limit early results. Wegovy doesn’t reach 2.4 mg until week 17.[2]
- Steady state: each step takes about 4 to 5 weeks to level off.[1]
- Biology: weight loss depends on eating less over time, so it adds up gradually and then levels off.
If you have to stop, the effects wear off over a few weeks. See how long semaglutide stays in your system, and our semaglutide maintenance dose guide for what happens after you reach your goal. More guides are on the semaglutide topic hub.
