Peptides for long COVID have no proven benefit yet. No peptide has improved long COVID in a completed, published randomized trial, and a 2026 review of 43 randomized trials of any long COVID treatment found that all but one offered only low-certainty evidence.[2] Thymosin alpha-1 has only lab data in long COVID, VIP (aviptadil) failed in severe acute COVID, and the most interesting peptide trials, of larazotide and tirzepatide, have not reported results.[6][7][9][10]

Long COVID is a chronic condition that can affect many organ systems and cause disability.[1] The lack of proven treatment pushes people toward products sold online. This guide grades the peptides by evidence, lists the registered trials we could verify, and explains what care looks like now. For related compounds, see the longevity and immune peptides hub.

What care looks like now

CDC describes long COVID as an infection-associated chronic condition lasting at least three months, with possible causes including organ damage, persistent inflammation, small-vessel problems, viral reservoirs and autoimmunity.[1] Because the causes differ between people, care is built around each person’s symptoms.

The largest care trial so far, STIMULATE-ICP in England, enrolled 1,152 adults in long COVID clinics. Overall, fatigue scores improved by about 4.5 points over 12 weeks, and the target was met in every group receiving structured care. Adding multi-organ MRI scans or digital rehabilitation made little extra difference.[3] Many people with long COVID also have post-exertional malaise; if that is you, pacing strategies from ME/CFS care may apply, as explained in our guide to peptides for chronic fatigue.

Prevention still counts: CDC says COVID-19 vaccination helps prevent long COVID, including in children.[1]

The peptides, one by one

Thymosin alpha-1

Thymosin alpha-1 is an immune peptide approved abroad, mainly for hepatitis B, and widely used for COVID-19 in China. The long COVID evidence is a single laboratory study: in blood cells from people with long COVID, thymosin alpha-1 shifted immune markers toward a more balanced pattern.[6] No one has tested it as a long COVID treatment in a trial.

The acute COVID data are not encouraging either. A 2023 meta-analysis of nine studies and 5,352 hospitalized patients found no effect on death and did not support its use.[5] FDA’s 2024 review concluded there was a lack of evidence for COVID-19 and noted online claims that it treats long COVID.[4] Our thymosin alpha-1 vs LL-37 comparison has the full picture.

VIP (aviptadil)

VIP is a natural lung-protective neuropeptide, and its drug form, aviptadil, was tested as an IV infusion in severe COVID-19. In the NIH-funded TESICO trial, 471 hospitalized patients with respiratory failure received aviptadil or placebo, and aviptadil did not improve recovery at 90 days.[7] We found no registered or published trial of VIP for long COVID. The nasal VIP promoted online comes from mold-illness protocols; see our VIP and CIRS guide.

Larazotide

Larazotide is an oral peptide designed to block zonulin and tighten the gut lining. One theory of post-COVID illness is that viral spike protein leaks from the gut into the blood. In a phase 2a trial of 12 children with MIS-C, a severe post-COVID inflammatory syndrome, those given larazotide cleared spike protein faster, had faster relief of gut symptoms and returned to usual activities sooner, with no drug-related adverse events reported.[8] A placebo-controlled phase 2a trial in 107 children and adults with long COVID completed in June 2026, but results had not been posted as of October 2026.[9] Larazotide is not approved anywhere, and its earlier phase 3 celiac trial was stopped.

Tirzepatide

Tirzepatide, an approved GLP-1 and GIP peptide drug, is the subject of the largest peptide trial in long COVID. The Long COVID Treatment Trial, run by Scripps Research, is randomizing about 1,000 people to tirzepatide or placebo; it is active but not recruiting, with completion expected around December 2026.[10] Until results are published, there is no evidence it helps long COVID symptoms.

Growth hormone

In a pilot study, 13 adults with neurologic long COVID symptoms and low growth hormone responses took daily growth hormone for nine months. Fatigue, mood and quality-of-life scores improved, but cognition, sleep and physical performance did not, and there was no placebo group.[11]

Safety concerns

Peptides bought online are not checked for identity, purity or sterility, and FDA has warned that several, including LL-37, may cause immune reactions or contain impurities.[12] For KPV, FDA found no human safety data by any route.[13] Approved drugs such as tirzepatide carry known side effects, mainly nausea and other gut symptoms, and should only be used under medical supervision.

Questions for your clinician

  • Which of my symptoms is most limiting, and what is our plan for it?
  • Do I have post-exertional malaise, and should I be pacing?
  • Have heart, lung, blood-clotting and autonomic causes been checked?
  • Is there a registered clinical trial I could join instead of trying an unproven product?

The bottom line

No peptide is proven for long COVID. Thymosin alpha-1 and VIP, the two most promoted, have no long COVID trial evidence, and VIP failed in acute disease. Real trials of larazotide and tirzepatide are the ones to watch. For now, the best-supported approach is coordinated, symptom-based care with your clinician.
