Quick answer: is your Ozempic pen empty? For practical purposes it is when the dose counter stops before your full dose as you turn the dose selector. That happens after 4 doses on the 1 mg and 2 mg pens, and after 4 doses of 0.5 mg (or 4 of 0.25 mg plus 2 of 0.5 mg) on the 2 mg/3 mL pen.[1] Also throw a pen away 56 days after first use, even if medicine is left.[1]

Seeing liquid in the window does not mean there is a dose left. The steps below come from Novo Nordisk’s instructions for use. For dosing in general, see our semaglutide dosage chart and semaglutide profile.

How many doses are in an Ozempic pen?

Each Ozempic pen holds 3 mL. The concentration, and so the number of doses, depends on the pen.[1]

PenDose you can dialDoses per penNeedles in the box
2 mg/3 mL (0.68 mg/mL)0.25 mg or 0.5 mg4 × 0.25 mg + 2 × 0.5 mg, or 4 × 0.5 mg6
4 mg/3 mL (1.34 mg/mL)1 mg only4 × 1 mg4
8 mg/3 mL (2.68 mg/mL)2 mg only4 × 2 mg4

On the starter pen, the first 4 weeks at 0.25 mg use half the pen, and the remaining half covers 2 weeks at 0.5 mg. Then you start a new 2 mg/3 mL pen at 4 doses of 0.5 mg.

The 2026 label also includes Ozempic single-dose prefilled syringes (0.25, 0.5 and 1 mg). These hold one dose each and are thrown away after use, so there’s no question of whether they’re empty.[1]

How to check how much is left

The pen’s dose counter is the only reliable gauge.[1]

  1. Turn the dose selector until it stops.
  2. Read the dose counter:
    • Starter pen: if it shows 0.5, at least 0.5 mg is left. If it stops before 0.5, there isn’t enough for a 0.5 mg dose. The same applies at 0.25.
    • 1 mg pen: if it shows 1, at least 1 mg is left.
    • 2 mg pen: if it shows 2, at least 2 mg is left.
  3. If it can’t reach your full dose, don’t use the pen. Use a new one.[1]

Don’t count clicks to measure a dose; the instructions say to rely only on the dose counter and pointer.[1]

The flow check (priming) for a new pen

Each new pen needs one flow check before its first injection, and only then.[1]

  1. Attach a new needle and remove both the outer and inner caps.
  2. Turn the dose selector to the flow-check symbol.
  3. Hold the pen needle up and press the dose button until the counter shows 0.
  4. A drop should appear at the needle tip.

If no drop appears, repeat the check up to 6 times. If there’s still no drop, change the needle and try once more. If that fails, don’t use the pen and call Novo Nordisk at 1-888-693-6742.[1]

The flow check uses a tiny amount of medicine, and multi-dose pens are filled to allow for it. The Wegovy label, for example, notes that its FlexTouch pen contains extra volume for priming.[2]

Why liquid is left over

A finished pen often still has some liquid inside. That’s expected. The pen stops you dialing a dose it can’t fully deliver, so whatever is left is less than one full dose, and part of the extra fill is there for the flow check.

The instructions are clear about what not to do:

  • Don’t use a syringe to draw medicine out of the pen.[1]
  • Don’t combine a partial dose from an old pen with a new pen. If there’s not enough for a full dose, start a new pen.[1]
  • Don’t keep using a pen past 56 days after first use, even with medicine left.[1]

Signs your pen didn’t deliver a dose

  • The counter doesn’t return to 0 while you press the button: the needle may be blocked or damaged, and you didn’t get any medicine. Change the needle and start over from step 1.[1]
  • A stream of liquid from the needle after you pull it out: the needle came out too soon, so you didn’t get the full dose. Count 6 seconds with the button pressed next time.[1]
  • A small drop at the tip after injecting is normal and doesn’t affect the dose.[1]

If you think you missed part of a dose, don’t inject again to make up for it without asking your prescriber. See our guide to missed GLP-1 doses.

Storage and the 56-day rule

Don’t freeze it, and don’t use it if it has been frozen. Store it with the cap on and no needle attached.[1] Traveling? See traveling with GLP-1 medications.

Check the pen before every use

Before each injection, look through the window. The medicine should be clear and colorless. If it’s cloudy or has particles, don’t use it.[1] Our guide to what color semaglutide should be explains why. Buy Ozempic only with a prescription from a state-licensed pharmacy. The FDA has found counterfeit Ozempic pens in the U.S. supply chain.[4]

How to dispose of an empty Ozempic pen

  • Put the used pen and needles in an FDA-cleared sharps container right after use.[1]
  • No sharps container? Use a heavy-duty plastic household container with a tight, puncture-resistant lid that stays upright, doesn’t leak and is labeled as hazardous waste.[1]
  • When it’s almost full, follow your community’s rules. Don’t put it in household trash unless local rules allow it, and don’t recycle it.[1][3]

Rotating where you inject helps too. See our GLP-1 injection sites guide. For cost questions about replacement pens, see Ozempic cost without insurance. More guides are on the semaglutide topic hub.
