Quick answer: “Ozempic breath” is the bad breath, sulfur taste or sour burps some people notice on semaglutide and similar drugs. It is not listed as its own side effect, but related ones are: burping affected 7% of adults on Wegovy 2.4 mg versus under 1% on placebo, and reflux 5% versus 3%.[1] The likely causes are slower stomach emptying, burping and reflux, dry mouth, and ketosis from eating very little.[2][3]

This guide explains each cause and what helps. For more on the drug, see our semaglutide profile.

Is bad breath a side effect of Ozempic?

Not officially. The labels do not list halitosis (bad breath), but they list several side effects that can cause it:[1][7]

A 2026 review of semaglutide and oral health lists halitosis, dry mouth, altered taste and enamel erosion from vomiting among the oral effects dentists and doctors may see, mostly as knock-on effects of the drug’s digestive actions.[3]

Cause 1: Food stays in the stomach longer

Semaglutide slows how fast the stomach empties. In a small placebo-controlled trial in women with obesity, semaglutide 1 mg left 37% of a solid meal still in the stomach four hours after eating, compared with none on placebo.[2] Food that sits longer can ferment, and the gases come back up as burps. Protein-rich and sulfur-containing foods (such as eggs, red meat, cruciferous vegetables, onions and garlic) can produce sulfur compounds that smell like rotten eggs.

Tirzepatide has a similar effect; we cover it in detail in our guide to semaglutide vs tirzepatide.

Cause 2: Burping and reflux

Burping and acid reflux carry stomach odors and acid into the mouth and throat. Both are listed side effects (see table above).[1] Reflux also leaves a sour or bitter taste. See our semaglutide heartburn guide for more.

Cause 3: Dry mouth

Saliva washes away bacteria and food. When it drops, odor-producing bacteria on the tongue and teeth thrive. A 2023 case series described three women on semaglutide for weight loss who developed marked dry mouth with low saliva flow; it improved with management that ranged from stopping the drug to saliva-stimulating treatment.[4] Eating and drinking less, a common effect of these drugs, can also leave the mouth drier. A review of halitosis lists dry mouth and digestive disorders among recognized causes of bad breath.[5]

Cause 4: Ketosis from eating very little

When food intake drops sharply, the body burns more fat and produces ketones, including acetone, which is exhaled. In a study of healthy volunteers, acetone built up during a day of fasting, and more so after high-fat eating.[6] Ketosis breath tends to smell fruity or like nail-polish remover rather than sulfurous. For most people on a GLP-1 this mild ketosis is not dangerous.

How to manage Ozempic breath

These steps target each cause. None has been tested in trials specifically for GLP-1 users.

Help your stomach:

  • Eat smaller meals and stop at the first sign of fullness.
  • Limit fatty and fried foods, which slow emptying further.
  • Notice whether eggs, red meat, garlic, onions or broccoli-family vegetables trigger sulfur burps, and cut back if so.
  • Avoid lying down for 2 to 3 hours after eating.
  • Limit fizzy drinks and alcohol, which add gas and reflux.

Help your mouth:

  • Drink water through the day.
  • Brush twice daily, floss, and clean your tongue.
  • Chew sugar-free gum or use sugar-free lozenges to boost saliva.
  • After vomiting, rinse with water and wait before brushing to protect enamel.
  • See your dentist, and tell them you take a GLP-1.[3]

Talk to your prescriber if symptoms are frequent. They may slow your dose increases, which the Wegovy label allows if side effects are hard to tolerate.[1]

Does it happen with tirzepatide too?

Yes, the same causes apply. Tirzepatide (Mounjaro, Zepbound) also slows stomach emptying, and Zepbound’s trials reported burping in 4% to 5% of people versus 1% on placebo, along with indigestion and reflux.[7] Many patients describe “sulfur burps” on tirzepatide as well.

How long does Ozempic breath last?

There is no published data on duration. Because burping, reflux and nausea are most common during dose escalation, many people find breath problems ease once they settle on a steady dose and adjust their eating. Dry-mouth symptoms in the case series improved with treatment.[4] If bad breath persists for weeks despite good oral care, a dentist can check for gum disease or other mouth causes.[5]

A note on protein shakes and supplements

Many people on GLP-1 drugs add protein shakes to protect muscle while appetite is low. Some find that large, rich shakes taken quickly worsen fullness and burping. Sipping smaller amounts over a longer time, or choosing lighter options, may be easier on a slow stomach. If a supplement seems to trigger sulfur burps, try a different type and see whether symptoms change.

When bad breath needs a doctor

Most Ozempic breath is a nuisance rather than a danger. See a clinician if you have:

  • Persistent foul burps with repeated vomiting, bloating or feeling full after small amounts, which can mean very slow stomach emptying. Wegovy is not recommended in severe gastroparesis.[1]
  • Heartburn that does not settle, trouble swallowing, or black stools.
  • Signs of dehydration, which can worsen dry mouth.
