Sources: NovoCare[1] and Wegovy.com[2] for self-pay prices, the White House for the list price,[3] and Novo Nordisk for the 2027 cut.[4] The $199 pen offer applies to people new to the Wegovy savings offer or NovoCare Pharmacy, for two fills through December 31, 2026.[1]

Pill vs pen: which costs less?

The pill is cheaper at every matching stage of treatment.

  • Starting: the 1.5 mg pill is $149, while the 0.25 mg pen is $199 for two months and then $349.
  • Maintenance: the 25 mg pill is $299, while the 2.4 mg pen is $349.
  • Higher dose: Wegovy HD 7.2 mg (approved in March 2026) is $399 and has no pill equivalent.[1]

Price is not the only factor. The pill must be taken every day on an empty stomach with a small sip of water, waiting before eating, while the pen is once a week.[5] Your clinician decides which form and dose suit you. See Wegovy pill cost for more on the tablet.

What a year of Wegovy costs without insurance

The pen dose is raised every 4 weeks, from 0.25 mg up to the 2.4 mg maintenance dose.[5] At current NovoCare prices, a new patient would pay roughly:

  • Months 1 and 2: 2 × $199 = $398
  • Months 3 to 12: 10 × $349 = $3,490
  • First year on the pen: about $3,888

On the pill, doses step up from 1.5 mg to 4 mg, 9 mg and then 25 mg, each for about a month before the next.[5] A rough first year:

  • Month 1 (1.5 mg): $149
  • Month 2 (4 mg): $199
  • Months 3 to 12 (9 mg then 25 mg): 10 × $299 = $2,990
  • First year on the pill: about $3,338

At list price, a year of Wegovy would cost more than $16,000.[3]

How the self-pay price fell

Wegovy’s cash price has dropped several times since 2025:

  • November 17, 2025: Novo cut the standard self-pay price of the pen from $499 to $349 and introduced a $199 starting offer.[6]
  • January 2026: the Wegovy pill launched at $149 a month for the starting dose.[2]
  • January 1, 2027 (announced): the list price falls to $675 a month for the 2.4 mg pen and 25 mg tablet. Novo said this does not change self-pay prices.[4]

How to buy Wegovy at the self-pay price

  1. Get a prescription from a licensed clinician.
  2. Ask them to send it to NovoCare Pharmacy, with your phone number and email.
  3. You get a text to set up your account and pay by card, Apple Pay, Google Pay, or FSA/HSA card.
  4. Choose free home delivery or pickup at a local pharmacy.[1]

Self-pay purchases do not count toward an insurance deductible or out-of-pocket limit.[1]

If you have Medicare or insurance you are not using

Medicare. Since July 1, 2026, the Medicare GLP-1 Bridge lets eligible Part D enrollees get Wegovy (pen or pill) for a $50 copay per month. You must meet BMI criteria, such as a BMI of 35 or higher, or 27 or higher with certain conditions, and get prior authorization. The program runs through December 31, 2027.[7]

Commercial insurance. If your plan covers Wegovy, the Wegovy savings card can bring the cost down to as little as $25 a month.

Self-pay vs list price: why the gap is so large

The list price is the starting point for wholesalers and insurers, not what most cash buyers pay. Novo Nordisk sets its self-pay prices separately, and they are flat: the pen costs the same at 0.25 mg as at 2.4 mg once the introductory fills are used up.[1] That makes budgeting simpler, because your monthly cost does not climb as your dose goes up.

A few practical points:

  • The $199 offer has a deadline. It covers two fills and must be used by December 31, 2026.[1]
  • You can pay with FSA or HSA money, which lowers your real cost if you contribute pre-tax.[1]
  • Prices change. The cash price has fallen more than once since 2025, so check before each refill.[6]
  • Cash spending does not count toward insurance totals. If you expect to meet a deductible this year for other reasons, ask your plan whether Wegovy could be covered first.[1]

Bottom line

Without insurance, expect to pay $349 a month for the Wegovy pen, $399 for Wegovy HD, or $149 to $299 for the pill through NovoCare. Check the Medicare GLP-1 Bridge if you have Part D.
