Quick answer: does insurance cover Wegovy? It depends on why it is prescribed and what your plan includes. Coverage for weight loss is optional for employers: in 2025, 19% of large firms offering health benefits covered GLP-1s for weight loss.[6] Coverage for heart risk reduction (approved March 2024) and MASH liver disease (approved August 2025) is more common, because those are treatments for a disease rather than weight loss alone.[2][3] Nearly every plan that covers Wegovy requires prior authorization.[10]

Wegovy contains semaglutide, the same drug as Ozempic. Its list price is $1,349.02 per package for the pen, the pill and the Wegovy HD pen.[16]
What Wegovy is approved for
Insurers start from the label. Wegovy injection is approved, alongside diet and exercise, for:[1]
| Use | Since | Who |
|---|---|---|
| Weight reduction and maintenance | 2021 | Adults and children 12+ with obesity; adults with overweight plus a weight-related condition |
| Lowering heart attack, stroke and cardiovascular death risk | March 8, 2024[2] | Adults with established heart disease and obesity or overweight |
| Noncirrhotic MASH with moderate to advanced fibrosis | August 15, 2025[3] | Adults (accelerated approval) |
The Wegovy pill is approved for weight management and heart risk reduction in adults, but not for MASH.[1]
The heart indication rests on the SELECT trial, where semaglutide lowered heart attack, stroke or cardiovascular death from 8.0% to 6.5% in 17,604 adults with heart disease and overweight or obesity.[4] The MASH indication rests on ESSENCE, where 62.9% on semaglutide had MASH resolution without worse scarring, versus 34.3% on placebo.[5]
Coverage for each use
Weight loss. Many plans exclude weight-loss drugs entirely, and employers choose whether to buy that coverage. KFF found 16% of firms with 200 to 999 workers, 30% with 1,000 to 4,999 and 43% with 5,000 or more covered GLP-1s for weight loss in 2025. About a third of covering firms required a dietitian visit or lifestyle program.[6]
Heart risk reduction. Plans that exclude weight-loss drugs may still cover Wegovy for this use. Aetna, for example, has separate criteria: a prior heart attack, stroke, symptomatic peripheral artery disease or revascularization; a BMI of 27 or higher; no type 2 diabetes; and ongoing standard heart treatment.[10]
MASH. Aetna’s 2026 policy covers the Wegovy injection for MASH with F2 to F3 fibrosis confirmed by testing or biopsy, prescribed by or with a gastroenterologist or liver specialist.[10]
Employer opt-outs and formulary changes
Cost has pushed some plans to drop weight-loss coverage:
- Blue Cross Blue Shield of Massachusetts excluded GLP-1s for obesity, including Wegovy, starting at plan renewals from January 1, 2026, unless an employer buys a rider. The exclusion cannot be appealed. Diabetes coverage continues.[9]
- CVS Caremark made Wegovy its preferred weight-loss GLP-1 on its main commercial template formularies on July 1, 2025, removing Zepbound. Members could ask for an exception if Wegovy did not work or was not tolerated.[7]
- Zepbound’s return: in May 2026, CVS Caremark said Zepbound would be added back to commercial formularies on October 1, 2026, and that Foundayo would be covered from June 1 for employers who choose it.[8]
Even when a pharmacy benefit manager lists Wegovy as preferred, your employer still decides whether weight-loss drugs are covered at all.[8]
Prior authorization basics
Typical criteria for weight management include:
- BMI thresholds. The Federal Employee Program requires a BMI of 30 or higher, or 27 or higher with heart disease or a weight-related condition such as high blood pressure.[11] Some plans set the bar higher: one Aetna policy requires a BMI of 35 or higher.[10]
- A lifestyle program. Aetna requires at least 6 months in a comprehensive weight management program before drug therapy.[10]
- Renewal at 5%. To continue, plans often require at least 5% weight loss from baseline after a set time on a stable dose.[10][11]
Plans may also treat the Wegovy pill, the standard pen and the higher-dose Wegovy HD pen differently. Aetna’s 2026 policy lists all three, but approves the pill for 6 months at first versus 8 months for the adult injection, and leaves Wegovy HD out of its heart-risk criteria.[10] Ask about the exact product your prescriber plans to use, and whether switching forms needs a new approval.

Medicare and Medicaid
Medicare. Since July 1, 2026, eligible Part D enrollees can get Wegovy for weight loss for $50 a month through the Medicare GLP-1 Bridge, until December 31, 2027.[12] If Wegovy is prescribed to reduce heart risk, the prescription goes to your regular Part D plan instead.[13] See does Medicare cover Ozempic for how the two paths work.
Medicaid. 13 state Medicaid programs covered GLP-1s for obesity as of January 2026.[14] See does Medicaid cover Ozempic.
How to check your plan
- Search the formulary on your member or pharmacy portal for “Wegovy.” Look for PA, step therapy and quantity limit flags.
- Ask about each use. If you have heart disease or MASH, ask whether Wegovy is covered for that use even if weight-loss drugs are excluded.
- Ask your HR or benefits team whether your employer covers weight-loss medicines, whether the plan is self-funded, and whether a change is coming at the next renewal date.
- Get the PA form your prescriber will need.
Paying less once covered
With commercial coverage, the Wegovy savings offer can reduce your cost to as little as $25, with up to $100 off a one-month fill. It is not available to people in Medicare, Medicaid, VA, DOD or TRICARE.[15] If your plan does not cover Wegovy, see how to get semaglutide without insurance, and for insurer-specific detail, does Aetna cover Wegovy and does BCBS cover Wegovy.
